TLC Incident Report

Please provide a detailed description of the incident/concern using specific concise, objective language. For matters involving violations of the student code of conduct, please provide as much detail as possible in describing the behaviors and activities that occurred. The use of direct quotes, including profanity or abusive language, is requested. Avoid using pronouns when multiple individuals are involved; use third person at all times. Please be thorough in your description regarding the circumstances that resulted in the report. These reports are submitted to The Learning Center Professional Staff for resolution.
